This bug tracks a survey that will be proposed to 3.0.19 users so as to collect feedback of people deciding not to upgrade to 3.6

This survey is for 9 locales (that account for 85% of our 3.0.x user base):

de, en-GB, en-US, es-ES, fr, ru, pl, pt-BR, it

I will file per locale bugs as dependencies.

We need this to be ready for March 29 (3.0.19 is March 30).
